This is the list of Heather-Renee' s prescriptions VS drugs found in her system
(blood collected was a grey top tube approx: 7 mL)
1. Ambien (Zolpidem) 10mg tablets #15 (NO REFILLS)
* she was given 2 free/ along with this RX

Prescribed to Heather-Renee' by Debbie Hoover, CNP
sig: 1 tab PO at bedtime for sleep
*empty bottle found in Bathroom of Michael Skarina's Residence on 09/18/05
Toxicology Report: 300 ng/mL present in blood
2. Lexapro (Escitalopram Oxalate) (Citalopram) 10mg tablets # 30 (5 refills before 6/20/05)

*empty bottle found in bathroom of Michael Skarina's residence on 9/18/05
Toxicology Report: 800ng/mL present in blood with metabolite present
3. Wellbutrin XL 150 mg ( Bupropion Hydrochloride) Sample bottle no prescription
according to Police report: Number issued was 7, but # remaining was 22 tablets
sig:1 tab PO daily
* empty bottle found on dresser in another room.
Toxicology Report: none found
4. Elavil (Amitriptyline) 25mg tablets # 30
*This bottle was observed by the Ball police dept at the time of death but was left behind, yet a photo of
the bottle is included in the crime scene photos.
It was not until Michael placed a call to Lt Carl Bordelon aprox a week later saying it was left behind
and he suspected she might have taken them that night.
(see Ball Police Dept report dated March 14, 2006 by Lt Bordelon to Det Stephans) Lt Borelon went to
Michael's home to pick up the bottle and according to the report, bottle was attached to said report, along with a recording
upon officer's arrival at scene.
Medication was prescribed to Michael Skarina
Heather-Renee's Toxicology Report: greater than 5000 ng/mL in blood
5. Nortriptyline (Pamelor)
*Not known where this drug came from no records of it being prescribed to Heather-Renee'
no prescription bottle noted.
Toxicology Report: 2000 ng/mL
6. Cyclobenzaprine (Flexeril)
*Not know where this drug came from, no records of it being prescribed to Heather-Renee'
no prescription bottle noted.
*I do know personally that Michael has been known to take this drug in the past.
Toxicology Report: less than 20 ng/mL
